Apparently there were quite a few of these unbreakable records on the Mayfair label, featuring “Lady In Blue” June Winters:

I wonder, is Hugh E. Perette really Hugo E. Peretti, of Hugo and Luigi fame?
It is, actually. Hugo and June were married until he died in 1986. She is still alive today and never remarried.
She died in September 2015 at age 96. I have the 78 but not the cover
