I used to ask him about the guy in the picture when I was a lad in the 70s. he says it looks like leprosy that the dude has. I'm not so sure loking at it now. that looks like some kind of pipe next to him. I doubt it was crack back in the 50s, but the drugs he's on could be the cause for the loss of his extremities.
